status of web UI for generating NAAN entries given a request email generated from the google form

JK: still beta; meant to be use with private github repo window open and editing
   -- candidate NAANs list
   -- actual registry

JK: asking everyone to try the form and check their email

MG: looks straightforward, but would like detailed instructions handy to explain some of the exceptional cases
JK: still ahead: need to reconcile this new procedure with README file on github
AC: if we had 6 people that would good, say one main and one secondary in any given month
MG: what's the problem with forwarding evidence?
JK: want recipient to feel like they're primary recipient, not an afterthought

Still to do:
- pre-validate entry against current NAAN registry
- work out procedures with github
